Our services are free and confidential to residents of Cuyahoga, Geauga, Lake, Lorain and Medina counties. For assistance, call 216/696-2719 extension 195, or mail Options@ltco.org. Options in Long Term Care is a 14-year-old program of the Long Term Care Ombudsman, supported by Lutheran Metropolitan Ministry and funded in part by United Way and the Deaconess Foundation. The Provider Database was created through the generous support of The Cleveland Foundation, Lutheran Church Missouri Synod World Relief Funds, and the Mt. Sinai Health Care Foundation.
