Person-centered care (PCC) is a philosophical approach to long term care that honors and respects the voice of elders and those working closest with them. It involves a continuing process of listening, trying new things, seeing how they work, and changing things in an effort to individualize care and de-institutionalize nursing homes, residential care facilities, assisted living facilities, and in long term care services(like home health care) provided in individuals' homes in the community.
In person-centered care the individual has the right to:
make decisions
have an individual plan of care
Be included on the care planning team (with the provider (such as the nursing home or home care program)
Have MY hopes, dreams and goals be central to my plan
